Please print off the following forms and complete to the best of your ability prior to your child’s first appointment. Please note that all forms must be completed by the child’s legal guardian. If you are unable to print the forms, you may contact the office to have them mailed to you and bring them to your first appointment. Alternatively, please arrange to attend your first appointment approximately 15 minutes early to allow for the completion of paperwork.
